End of Life Planning

Our Meeting offers this worksheet to help Friends consider their wishes for the end of life and gather important information in one place.

It includes space for contacts, burial or cremation wishes, and arrangements for a memorial meeting or service. You can also note where your will and other documents are kept, along with any personal requests you would like others to know.

You may find it helpful to talk through the questions with family, friends, or others who may help carry out your wishes. There is no need to complete it all at once.

The worksheet is available to all, whether or not you are part of our Meeting.
